Choose which phone # you would like to have as your ID, enter this ID # in the form above.

Include your ID # & the words; 'Tab Affiliate' in all emails to us.

It will be your responsibility to keep track of your sales.

Email us your 'Purchaser's' information -
         Name, Address, Number of Packages, Free Packages, (at S&H charges only).

Set up a PayPal Account, this is how you will be paid by us. PayPal is Free to join.

         'The Tab' sells for $9.95 per package of 4 Tabs.
         As an Affiliate, you receive 50% of the sale.
         This equates to $4.98 for each package sold by you.
         You, as Affiliate, do not receive a portion of S&H.

         When a customer orders a second package at S&H only, ($2.50)
         due to this being a promotional item, you will receive 50% of this also
         which equates to an additional $1.25.

         This is a very 'HOT' item to promote. This is a PERFECT GIFT

Affiliate Payments are processed every-other Friday for a 2-week period,
with a 1-week processing time.

This means;
         If you are paid on Aug 7th, you are being paid for sales during
         the period of Jul 19th through Aug 1st.
               If you are paid on Aug 21st, you are being paid for sales during
         the period of Aug 2nd through Aug 15th.
